Forms for employers

You can find a selection of our forms here. The forms are divided into different categories.

Signature

You have to sign the form by hand before you post it to Försäkringskassan. So you cannot send in the form by e-mail.

How do I download forms?

Our forms are available for download in PDF format. To view the forms, you must have the free program Adobe Reader in your computer. If you fail to open forms, it may be because you need a newer version of Adobe Reader or your web browser. You can download the program for free on the Internet.

Use form 6210 if you are a sole proprietor and will do work in another EU/EEA country or Switzerland. You need to obtain a certificate regarding applicable legislation in order to continue to belong to the Swedish social insurance.
